NHS cervical screening programme decline

Use this service to opt out of the cervical screening programme.

You can use this service if you:

  • are registered at the surgery

Women aged 25 to 64 automatically get invited to cervical screenings (also known as ‘smear tests’). Regular screenings greatly lower the risk of getting cervical cancer.

You can decide if you want to get screening invites and reminders or not. If you don’t want to get invites and reminders, you can opt-out of the NHS cervical screening programme.

As a practice we have tried to accommodate the various difficulties patients have with having a smear by offering a variety of services, which include offering smears during extended hours clinics and offering smear only nurses clinics.

Before you start

We’ll ask you for:

  • your first and last name, date of birth, sex, postcode, email and phone number
  • if applicable, the details of the person you are completing the form on behalf of

We would strongly advise that you take time to view the Jo’s Trust website for information regarding cervical screening and the risks involved in not participating in the NHS cervical screening programme. If you aren’t able to view this website or you feel you would benefit from talking to one of our practice nurses please speak to a receptionist to arrange a routine nurse’s appointment.
