Private (Non NHS Services)

Certain services such as insurance forms, passport applications, private notes, insurance or pre-employment/student medicals and travel cancellation certificates are not covered by the NHS. Charges will be made to the individual or company involved. This type of work cannot be prioritised over other NHS work and therefore please do not expect the surgery to be able to return forms to you in a short space of time- please do not expect any non-NHS work to be completed in less than 4 weeks. The GP's have very high workloads and therefore these pieces of work can take up to 2 months to be completed. We appreciate that at time forms can be time limited, however we cannot make guarantee's but will do our best.

Test Results

Please allow 5 working days before calling for your test result. Please ring 0208 971 5640 between 14.00pm - 16.00pm.

Test results should be requested in person.

You can now register online to view your test results.

Blood Tests

If you have been given an Epsom and St Helier Blood test form you are welcome to have your blood taken at the surgery every morning except Tuesdays between 8:30pm and 12:30pm.

If you have a St Georges Hospital Trust blood test form you can have your blood taken at the Nelson Hospital on Kingston Road at the following time:

Monday- Friday8am – 7.45pm

Saturdays 8am – 1.45pm

Access to Medical Records

If you require access to your medical records please complete the form below and bring it in to the surgery with a valid proof of ID, initial requests can be made over the phone but we will need you to come into the surgery with photographic ID before we can process a request for medical records. There is no longer a standard charge for accessing your medical records as the legislation on this has now changed (as of 25th May 2018) we will charge administration fee's if the request is 'excessive, repetitive or manifestly unfounded'.

The practice will in most cases have records available within 1 months of receiving the request for access to records, please be as specific as possible about what it is you would like to know about your medical history as it means we can process specific requests more efficiently than entire medical records which take a lot of GP time to process.

In order to be as cost and time efficient as possible we will be offering medical records electronically predominantly via email, the cost and time involved in printing off and collating medical records is extensive, although if you are unable to access records electronically we will do our best to accommodate specific needs.
