Texts are one of the most cost effective and time efficient ways of communicating in bulk with our patients, not only do texts reduce the number of incidences of people not turning up for pre-booked appointments because they have forgotten but research shows that the use of texts over letters improves patient engagement and uptake of services. It is also the preferred way of receiving invites and reminders by many of our patients so please ensure we can still text you and let us know you are happy to receive texts from the surgery, please complete this form and email it in or bring it in.
If you previously replied to the text that came out asking for your consent, there should be no need to complete this form, however if your mobile number is attached to more than 1 patient at the surgery then it will only have consented to one of those people not all of you. If this is the case please fill in the form for all people linked to a mobile number.
Also a point to note is we cannot add consent for patients who are over 16 with a parents mobile number, we will need the patient's own mobile phone number or for them to come in and speak to reception about allowing a parent to still receive messages on their behalf.
