Summary Care Records

Using our online patient service called Patient Access, you can:

  • View parts of your GP record, including information about medication, allergies, vaccinations, previous illnesses and test results
  • View clinical correspondence such as hospital discharge summaries, outpatient appointment letters and referral letters

Transferring your electronic health record

Your GP practice holds copies of your patient health record electronically and in paper format. Both contain the healthcare information about you that your GP needs including your medical history, medications, allergies, immunisations and vaccinations.

If you have previously registered with a different GP in England, upon registering at this practice your electronic health record will, where possible, be transferred automatically from your previous practice through the use of an NHS system called GP2GP.

Access to your records

There is no longer a standard charge for accessing your medical records as the legislation on this has now changed (as of 25th May 2018) we will charge administration fee’s if the request is ‘excessive, repetitive or manifestly unfounded’.

The practice will in most cases have records available within 1 months of receiving the request for access to records, please be as specific as possible about what it is you would like to know about your medical history as it means we can process specific requests more efficiently than entire medical records which take a lot of GP time to process.

In order to be as cost and time efficient as possible we will be offering medical records electronically predominantly via email, the cost and time involved in printing off and collating medical records is extensive, although if you are unable to access records electronically we will do our best to accommodate specific needs.

If you are moving abroad, please make sure you request copies of your medical records more than a month in advance of you leaving the country.
