New patient registration

Use this service to register with the surgery.

Please be aware it can take up to 7 working days for a new patient registration to be processed.

We will contact you if there is an issue with your registration, otherwise please assume that you are registered following the 7 days.

You can use this service if you:

  • want to register at the surgery
  • live within our catchment area

Before you start

We’ll ask you for:

  • your first and last name, date of birth, sex, postcode, email and phone number
  • if applicable, the details of the person you are completing the form on behalf of
  • your NHS number
  • details of previous GP surgery
  • basic health and medical information
